Electronic Disclosure and Consent Agreement
By agreeing to do business electronically with Citizens State Bank you consent to the electronic delivery of the disclosures, agreements, change notices, terms and conditions, statements and any other documents related to the account(s) you have applied for or opened. You also agree that we do not need to provide you with additional paper (non-electronic) copies of the disclosures, agreements, change notices, terms and conditions and any other documents, unless specifically requested. Once you consent to the electronic disclosures, your request for paper copies may result in a fee. Please contact us for more information about the fee.
Electronic Delivery of Disclosures and Notices
In order to use the online Loan Application and to access, receive and retain the disclosures, agreements, change notices, terms and conditions, statements and any other documents, you must provide, at your own expense, an internet connected device that is compatible with Citizens State Bank's Online loan/account application deployed at the time of access. Your device must meet the minimum requirements outlined below. You also confirm that your device will meet these specifications and requirements and will permit you to access and retain the disclosures, agreements, change notices, terms and conditions, statements and any other documents electronically each time you access and use online products.
System Requirements to Access Information
To receive an electronic copy of the disclosures, agreements, change notices, terms and conditions, statements and any other documents you must have the following equipment and software:
A personal computer or other device which is capable of accessing the Internet. Your access to this page verifies that your system/device meets these requirements.
An Internet web browser which is capable of supporting 128-bit or better encryption, like Microsoft Internet Explorer or Google Chrome. It is recommended to always use the latest version of your web browser. Your access to this page verifies that your browser and encryption software/device meets these requirements.
You must have software which permits you to receive and access Portable Document Format or “PDF” files, such as Adobe Acrobat Reader. Your ability to electronically sign this document using Adobe Sign verifies your system/device has the necessary software to permit you to access PDF files.
Withdrawal of Electronic Acceptance of Disclosures and Notices and Requesting Paper Copies
You may withdraw your consent to receive disclosures, agreements, change notices, terms and conditions, statements and any other documents, in electronic form for any of your accounts by contacting us via email using the “Contact Us” link on our website www.mycsbin.com or by telephone at 765-529-5450 or by postal mail at Citizens State Bank, 1238 Broad Street, PO Box C, New Castle, IN 47362. We may treat your provision of an invalid email address or the subsequent malfunction of a previously valid address or cancellation of participation in our online banking product as a withdrawal of your consent to receive electronic statements or documents. Any withdrawal of your consent to receive electronic statements will be effective only after we have a reasonable period of time to process your withdrawal.
You are not required to receive notices or disclosures or sign documents electronically and may request paper copies of documents or disclosures if you prefer to do so. Termination/Changes Regarding Electronic Delivery of Documents
We reserve the right, in our sole discretion, to discontinue the provision of your electronic Communications, or to terminate or change the terms and conditions on which we provide electronic Communications. We will provide you with notice of any such termination or change as required by law.
Bank shall not be responsible or liable for:
- Consequential or incidental damage for negligent performance by BANK of its SERVICES
- Delayed delivery, non-delivery, wrong delivery of an e-mail due to unpredictability of the Internet. Inaccurate content except at the originating time of e-mail.
- Security associated with the information transmitted via e-mail
- Damages arising from unauthorized access to SERVICES
- Any costs associated with updating, modifying or terminating CUSTOMER’s software or hardware.
- Any charges from your internet service provider for online time or any other charges related to accessing and/or viewing such documents
How to Update Your Records
It is your responsibility to provide us with true, accurate and complete email address, contact, and other information related to this Disclosure and your account(s), and to maintain and update promptly any changes in this information. You can update information such as your email address through your online banking using "Netteller Account Settings" or by telephone at 765-529-5450 or by postal mail at Citizens State Bank, 1238 Broad Street, PO Box C, New Castle, IN 47362. In order to change your mailing address, Citizens State Bank requires that you visit a branch or contact us by phone at 765-529-5450. Citizens State Bank will require that you display valid identification and or answer questions so that we may verify your identity.
By agreeing to this Consent, I understand that (i) electronically signing and submitting any document(s) to Citizens State Bank legally binds me in the same manner as if I had signed in a non-electronic form, and (ii) the electronically stored copy of my signature, any written instruction or authorization and any other document provided to me by Citizens State Bank, is considered to be the true, accurate and complete record, legally enforceable in any proceeding to the same extent as if such documents were originally generated and maintained in printed form. I agree not to contest the admissibility or enforceability of Citizens State Bank’s electronically stored copy of this Consent and any other documents.
By using the System to electronically sign and submit any document, I agree to the terms and conditions of this Consent.
